Health Coverage Tax Statement Notice

Mt. Baker Products, Inc. offers health insurance coverage to eligible employees. As required by the Internal Revenue Service, this notice informs full-time employees of their right to receive a copy of their Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for any year in which they were offered coverage.

You are not required to have this statement to file your federal income tax return. However, if you would like a copy of your Form 1095-C for a specific coverage year, please visit our Contact page and reach out by phone or message. Please note “1095-C Request” and specify the coverage year, along with your full name and the last four digits of your Social Security number, so we can locate your record.

We will furnish your statement within 30 days of receiving your request.

This notice is maintained on an ongoing basis to satisfy IRS notice requirements under §6056(c)(3) of the Internal Revenue Code for each applicable coverage year.
